Read and sign both documents
Complete the top portion and sign below where it asks for “Applicant Signature.” REQUIRES PAR./GUARD. SIGNATURE
Fill out Part II and sign on the the top of page 2. Print only pages 2-3, front+back. REQUIRES PAR./GUARD. SIGNATURE
Please have two references filled out by someone you have known for over a year and who is at least 21 years of age (family, teachers, neighbors, etc.)
High School Volunteers
Return all completed information to your Guidance Counselor or to the Big Brothers Big Sisters office at 2901 Langhorne Road.
